Luke O'Brien

Dr. Luke O'Brien is originally from Columbus, Ohio. He is a graduate of Cedarville University, where he earned his Bachelor of Science in Pharmaceutical Science as well as his Doctor of Pharmacy degree. Luke is currently in his first year of the two-year Pharmacy Informatics Residency. His professional interests include Pharmacy Automation, Data Analytics, and Medication Safety. In his free time, Dr. O'Brien enjoys photography, video games, and swing dancing. Dr. O'Brien would like to thank God, his wife, family, friends, and mentors for their unwavering support and patience.

Tiffany Dinh, PharmD, MPH

Dr. Tiffany Dinh is originally from Alameda, California. She earned her Bachelor of Science in Nutritional Sciences: Toxicology at the University of California, Berkeley, followed by a Doctor of Pharmacy degree from The University of Texas at Austin College of Pharmacy and a Master of Public Health at UTHealth Houston School of Public Health. Dr. Dinh is currently in her first year of the two-year Pharmacy Informatics Residency. Her professional interests include digital health, optimization of clinical decision support tools, and pharmacy technology/automation. In her free time, she enjoys weightlifting, listening to podcasts, and volunteering. Dr. Dinh would like to thank her family, friends, and mentors for their steadfast encouragement, guidance, and support.

Deanna Guan, PharmD

Dr. Deanna Guan is originally from San Francisco, California. She earned her Doctor of Pharmacy degree from the University of California, San Francisco. Dr. Guan is currently in her first year of the Investigational Drugs and Research Pharmacy Residency Program. Her professional interests include oncology, pharmacogenomics, clinical trial design and operations, and advancing patient-centered care in underserved communities. In her free time, she enjoys exploring new restaurants and coffee shops, reading, and traveling. Dr. Guan is deeply grateful to her family, friends, and mentors for their unwavering support and encouragement throughout her journey.

Eric Mackin, PharmD

Dr. Eric Mackin is originally from Louisville, Kentucky. He earned his Bachelor of Arts degree in Psychology from Bellarmine University, followed by his Doctor of Pharmacy degree from the University of Kentucky. Dr. Mackin is currently in his first year of the two-year Medication Use Safety and Policy residency. His professional interests include medication safety, Just Culture, formulary management, and pharmacy policy. In his free time, he enjoys playing volleyball, reading, and trying local coffee shops. Dr. Mackin would like to thank his family, friends, and mentors for their unwavering love, support, and guidance.

Mojibola Awe, PharmD

Dr. Moji Awe is originally from Lagos, Nigeria and then moved to Columbia, South Carolina. She earned her Doctor of Pharmacy degree from the University of South Carolina College of Pharmacy and completed her PGY1 Pharmacy Residency at Emory University Hospital in Atlanta, Georgia. Dr. Awe’s professional interests include transplant, patient education, and academia. After completion of her PGY2 Ambulatory Care Residency, she plans to practice in an area of interest, complete her Master of Business Administration degree, and become an educator of pharmacy students. In her free time, Dr. Awe enjoys cooking, baking, exploring new restaurants, reading, attending concerts, and traveling. Dr. Awe would like to thank her family, friends, and mentors for their continued support and guidance!

PGY-2 Cardiology

John Fattal, PharmD

Dr. John Fattal is originally from Wayne, New Jersey. He earned his Doctor of Pharmacy degree from Albany College of Pharmacy and Health Sciences and completed a PGY1 pharmacy residency at The Johns Hopkins Hospital. Dr. Fattal’s professional interests include cardiac critical care and advanced heart failure. His ultimate career goal is to practice as a board-certified clinical specialist in a cardiac ICU, with opportunities to participate in research and precepting. Outside of pharmacy, he enjoys videogames, watching the Green Bay Packers, and spending time with family, friends and his two cats, Kirby and Petal. Dr. Fattal would like to thank his family, girlfriend, friends, and mentors for their support and guidance at every step of the journey.

PGY-2 Critical Care

Thomas Diaz, PharmD

Dr. Thomas Diaz grew up in Concord, North Carolina and earned his Bachelor's degree and Doctor of Pharmacy degree from The University of North Carolina at Chapel Hill. Dr Diaz's professional interests include critical care, emergency medicine, and aerospace medicine. Upon completion of his PGY-2 Critical Care Pharmacy Residency, he plans to pursue critical care board certification (BCCCP) and practice at an academic medical center with opportunities to precept students/residents while engaging in research. Ultimately, he hopes to work for NASA or a commercial spaceflight program, conducting research and overseeing their pharmacy operations. In his free time, he enjoys playing golf, snowboarding, watching sci-fi movies, exploring new restaurants, and reading. Dr. Diaz would like to thank his family, friends, co-residents, and mentors for their ongoing support.

Alexander Coughlin, PharmD

Dr. Alexander Coughlin is originally from Valrico, Florida. He earned his Doctor of Pharmacy degree from the University of South Florida Taneja College of Pharmacy and went on to complete his PGY1 Pharmacy Residency at Tampa General Hospital. Dr. Coughlin’s professional interests include antimicrobial stewardship, opioid use disorder, and trauma management. Upon completing his PGY2 Pharmacy Residency, he plans to pursue emergency medicine board certification and practice as a clinical specialist at an academic medical center. In his spare time, he enjoys thrifting, spending time with friends, cooking, and attending sporting events. Dr. Coughlin would like to sincerely express his gratitude to his family, friends, and mentors for their unwavering support and encouragement.

Catherine Smith, PharmD

Dr. Catherine Smith is originally from Stow, Massachusetts. She earned her Bachelor of Science in Pharmaceutical Sciences and her Doctor of Pharmacy from the University of Pittsburgh, and went on to complete her PGY1 Pharmacy Residency at Brown University Health Rhode Island Hospital. Upon completion of her PGY2 Infectious Diseases Pharmacy Residency, she aims to pursue board certification and practice as a clinical specialist in infectious diseases at an academic medical center. She hopes to precept and teach students and residents, and participate in research in infectious diseases practice and public health. Outside of ID and pharmacy, she loves reading, dogs, live music, and the ocean! Dr. Smith would like to thank her friends and family for their support, as well as her wonderful preceptors and mentors for their investment in her career!

Debika Kundu, PharmD

Dr. Debika Kundu is originally from the Bronx, New York. She earned her Doctor of Pharmacy degree from St. John’s University College of Pharmacy and Health Sciences and completed her PGY-1 Pharmacy Residency at North Shore University Hospital in Manhasset, New York. Dr. Kundu’s clinical interests include primary care, oncology, solid organ transplant, and academia. She aspires to become board-certified and practice as a clinical pharmacy specialist at a large academic medical center, where she hopes to precept and mentor pharmacy students and residents. In her free time, she enjoys hiking, practicing Pilates, cooking new recipes, and reading novels. She would like to thank God, her family, friends, and mentors for their unwavering support and guidance.

Grace Ukazim, PharmD

Dr. Grace Ukazim is from Katy, Texas. She earned her Doctor of Pharmacy degree from The University of Texas at Austin and completed her PGY-1 Pharmacy Residency at the University of Maryland Baltimore Washington Medical Center. Dr. Ukazim’s professional interests include pain management, substance use disorders, and palliative care. Upon completion of her PGY-2 Pharmacy Residency, she plans to practice as a board-certified clinical specialist and precept students. In her free time, she enjoys reading, spending time with loved ones, going on walks, and traveling. Dr. Ukazim would like to thank her support system, including her family, friends, and mentors, for their constant encouragement and guidance.

Lauren Prack, PharmD

Dr. Lauren Prack is from Whitefish, Montana and earned her Doctor of Pharmacy degree from Duquesne University in Pittsburgh, PA before completing her PGY1 Pharmacy Practice Residency at The Johns Hopkins Hospital. Dr. Prack’s professional interests include emergency medicine and pediatrics. Upon completion of her PGY2 Pediatric Pharmacy Residency, she plans to practice as a board-certified clinical specialist at an academic medical center. Outside of pharmacy, she enjoys pilates, baking, and her dog Jules. Dr. Prack would like to thank her family, friends, and mentors for their endless support and encouragement!

Madison Anderson, PharmD

Dr. Madison Anderson is originally from Akron, Ohio. She earned her Doctor of Pharmacy degree from Ohio Northern University Raabe College of Pharmacy and completed her PGY1 Residency at The Johns Hopkins Hospital. Dr. Anderson’s professional interests include psychiatry and academia. Upon completing her PGY2 Psychiatric Pharmacy Residency, she plans to pursue board certification in psychiatry. Ultimately, she hopes to practice as a clinical specialist at an academic medical center, where she will have the opportunity to precept students and residents. In her free time she enjoys baking, hiking, and walking her dog. Dr. Anderson would like to sincerely thank her friends, family, and mentors for all of their support!

PGY-2 Thrombosis and Hemostasis Management

Razan Fannoun, PharmD

Dr. Razan Fannoun grew up in Amman, Jordan and earned her Doctor of Pharmacy degree from The University of Jordan. She completed her PGY-1 residency at Cleveland Clinic Akron General. Her professional interests include thrombosis and hemostasis management in high-acuity settings, critical care, and pharmacy informatics. She is particularly interested in the intersection of technology and pharmacotherapy and hopes to contribute to the optimization of medication use systems for high-risk therapies. After residency, she plans to pursue a clinical specialist role at an academic medical center with opportunities to teach, precept, and contribute to research. Outside of pharmacy, she enjoys baking sourdough bread, gardening, and spending time with her husband and two cats, Goose and Astro. She is grateful to the mentors, colleagues, and loved ones who’ve helped shape both her practice and her path.
