Becoming a patient can be an isolating and overwhelming experience. Receiving peer-to-peer support can help patients feel better, adopt positive behaviors, and improve their health. Peer-to-peer support involves connecting a patient with others with a similar health condition, people “who have been there.” The Roadmap to Peer Support provides information on how to build and maintain a peer support program.
This guide developed through two-year project funded by the Patient-Centered Outcomes Research Institute organizes the steps to creating a peer support guide into four parts: plan, implement, assess, and sustain and provides resources to help you on your way.
