We’ll ask you to provide us with your most recent health insurance information when you call to schedule an appointment for your child and when you arrive for your child’s appointment. This helps to ensure the billing process goes as efficiently as possible.

If your insurance plan requires a referral or authorization for treatment, we ask that you provide that information to us as soon as possible. You may need to send or bring other forms with you for your child’s appointment, depending on the care he or she is receiving. The member of the Johns Hopkins All Children’s team who is working with you to schedule the appointment can answer additional questions about necessary forms.
