I hereby attest that I have completed the required ACD-ABA training course and that I understand the ACD-ABA key concepts below:

• I understand the ACD requirements that include ABA provider requirements.
• I understand correct billing practices/claims filing, authorizations, exclusions, and medical records documentation for the ACD-ABA program.
• I understand provider responsibilities and program requirements of the ACD-ABA program.
• I understand basic TRICARE rules and 32 CFR 199.
