Explanation of Change:

Johns Hopkins Health Plans has added new codes to the Laboratory Developed Tests list for USFHP providers that was published June 30, 2026.

The following codes moved from Not Covered (NC) to Prior Authorization (PA) required on June 30, 2026:

  • 81226 - molecular genetic testing of the CYP2D6 gene, which is located on chromosome 22
  • 81230 – analysis of common variants of the cytochrome technical laboratory test performed by a lab analyst to analyze common variants of the cytochrome P450 family 3 subfamily A member 4 gene (CYP3A4), such as the *2 and *22 alleles 
  • 81247 - genetic analysis of the glucose‑6‑phosphate dehydrogenase (G6PD) gene, focusing on common variants such as A and A‑ 
  • 81418 - pharmacogenomics genomic sequence analysis panel that tests at least six genes, including CYP2C19, CYP2D6, and CYP2D6 duplication/deletion, to guide drug metabolism and therapy decisions
  • 0026U - ThyroSeq® version 3 proprietary laboratory test for thyroid cancer risk assessment using next-generation sequencing of 112 genes
  • 0376U - ArteraAI Prostate Test, a proprietary AI-based diagnostic test for prostate cancer prognosis
