Protecting the privacy and security of your information is fundamental to the mission of Johns Hopkins Health Plans (“JHHP,” “we,” “our,” or “us”). We understand that our members trust us with sensitive personal and health information, and we are committed to protecting that information while providing secure, convenient, and reliable digital services.
This Member Mobile Application Privacy Notice (“Privacy Notice”) explains how Johns Hopkins Health Plans collects, uses, shares, protects, and retains information when enrolled members and their Authorized Representatives use the Johns Hopkins Health Plans Mobile Application (“Mobile Application”). It also explains the choices available to you regarding certain information collected through the Mobile Application and the steps Johns Hopkins Health Plans takes to safeguard your information.
This Privacy Notice applies only to information collected through the Mobile Application and related digital services. It supplements the Johns Hopkins Health Plans Notice of Privacy Practices by describing the privacy practices that apply specifically to the Mobile Application. It does not replace the Notice of Privacy Practices. To the extent this Privacy Notice addresses information that constitutes Protected Health Information (“PHI”) under the Health Insurance Portability and Accountability Act (“HIPAA”), the Johns Hopkins Health Plans Notice of Privacy Practices governs the use and disclosure of that information.
By creating a Mobile Application account or using the Mobile Application, you acknowledge that you have had the opportunity to review this Privacy Notice. Your use of the Mobile Application is also subject to the applicable Mobile Application Terms of Use and other agreements governing your relationship with Johns Hopkins Health Plans.
Johns Hopkins Health Plans recognizes that protecting the privacy and security of your information is essential to the trust you place in us. We are committed to maintaining that trust by protecting your information, complying with applicable privacy and security requirements, and continually improving the Mobile Application and the digital services we provide.

This Privacy Notice applies to individuals who access the Johns Hopkins Health Plans Mobile Application after successfully completing the account registration and identity verification process. Access to the Mobile Application is limited to enrolled members of health plans administered by Johns Hopkins Health Plans and to individuals who have been properly authorized and verified to act on behalf of an enrolled member.
The Mobile Application supports digital services made available through participating health plans administered by Johns Hopkins Health Plans. Available features and functionality may vary depending on the Member's health plan, line of business, and applicable legal or contractual requirements.
This Privacy Notice applies only to information collected, displayed, maintained, or transmitted through the Mobile Application. It does not apply to other Johns Hopkins Health Plans websites, applications, member portals, or services unless specifically stated. o text in field]

To help make this Privacy Notice easier to understand, the following terms are used throughout this document.
Member means an individual enrolled in a health plan administered by Johns Hopkins Health Plans who is eligible to access the Mobile Application.
Authorized Representative means an individual whose identity and authority have been verified by Johns Hopkins Health Plans to act on behalf of a Member in accordance with applicable law, health plan requirements, and Johns Hopkins Health Plans policies. Depending on the circumstances, an Authorized Representative may include a parent, legal guardian, personal representative, health care agent, an individual acting under a valid power of attorney, caregiver, or another individual authorized by the Member or otherwise permitted by law.
Mobile Application means the Johns Hopkins Health Plans mobile application and all related digital services, features, and functionality made available through that application.
Personal Information means information that identifies, relates to, describes, or can reasonably be linked to an identified or identifiable individual. Personal Information includes information collected through the Mobile Application that is not Protected Health Information, as well as information that may also constitute Protected Health Information under applicable law.
Protected Health Information (PHI) means individually identifiable health information protected by HIPAA and other applicable laws governing the privacy and security of health information.

The Mobile Application is intended exclusively for enrolled Members and Authorized Representatives. Before access is granted, all users must successfully complete the account registration process established by Johns Hopkins Health Plans. To protect the privacy and security of Member information, Johns Hopkins Health Plans uses identity verification and authentication measures designed to confirm that individuals requesting access are authorized to do so.
Members are responsible for maintaining accurate account information and protecting the confidentiality of their usernames, passwords, authentication codes, and other account credentials. Members should not share their account credentials with another individual unless access has been established through an Authorized Representative process recognized by Johns Hopkins Health Plans.Authorized Representatives
A Member may authorize another individual to act on the Member’s behalf in accordance with applicable law and Johns Hopkins Health Plans policies. Before access is granted, Johns Hopkins Health Plans will verify the identity of the Authorized Representative and, when appropriate, the individual’s legal authority to act for the Member. Johns Hopkins Health Plans may require documentation reasonably necessary to verify an individual’s authority before granting or continuing access to a Member’s information. Verification requirements may vary depending on the services requested, applicable legal requirements, and the nature of the representative’s authority.
Once access has been approved, an Authorized Representative may view information and perform only those activities permitted by the Member’s authorization, applicable law, and Johns Hopkins Health Plans policies. Access may be limited based on the representative’s authority and the services available through the Mobile Application.
Members remain responsible for notifying Johns Hopkins Health Plans if an Authorized Representative’s authority changes, expires, or should be revoked. Johns Hopkins Health Plans reserves the right to modify, suspend, or terminate an Authorized Representative’s access if authorization is withdrawn, verification requirements are no longer satisfied, continued access would be inconsistent with applicable law or Johns Hopkins Health Plans policies, or doing so is necessary to protect the privacy, security, or integrity of Member information. -
The information collected through the Mobile Application depends on how you use the services available to you and the features you choose to access. Some information is provided directly by you when you create or manage your account, while other information is generated automatically as you interact with the Mobile Application. Johns Hopkins Health Plans collects the information reasonably necessary to provide Mobile Application services, administer your health plan, protect your account, improve digital services, and comply with applicable legal and regulatory requirements.
Information You Provide
When you create an account, update your profile, communicate with Johns Hopkins Health Plans, upload documents, complete forms, or otherwise interact with the Mobile Application, you may provide information such as your name, mailing address, email address, telephone number, date of birth, Member identification number, username, and other information necessary to verify your identity or provide requested services. Depending on the features you use, you may also choose to submit documents, photographs, secure messages, or other information through the Mobile Application.
Health Plan Information
The Mobile Application provides secure access to information associated with your health plan membership. Depending on your coverage and the services available through this Mobile Application and your plan, this information may include eligibility and enrollment information, benefit information, claims, explanations of benefits, pharmacy information, prior authorization information, care management information, provider information, and other records maintained in connection with your health plan.
Some information available through the Mobile Application constitutes Protected Health Information and is also governed by the Johns Hopkins Health Plans Notice of Privacy Practices.
Technical Information About Your Device
When you access the Mobile Application, Johns Hopkins Health Plans automatically receives certain technical information to help operate, secure, and improve digital services. This information may include your device type, operating system, Internet Protocol (IP) address, Mobile Application version, browser type where applicable, language preferences, authentication activity, diagnostic information, error reports, and other information necessary to maintain the performance, availability, and security of the Mobile Application.
Johns Hopkins Health Plans uses this information to maintain system security, troubleshoot technical issues, improve Mobile Application performance, support cybersecurity activities, and better understand how Members use available features. Technical information is not used to make decisions regarding your eligibility for coverage, benefits, or medical care.
How You Use the Mobile Application
Johns Hopkins Health Plans also collects information about how users interact with the Mobile Application, including the features accessed, pages viewed, documents uploaded, secure messages transmitted, and actions taken while signed into an account. This information helps evaluate the effectiveness of digital services, improve the Member experience, identify technical issues, maintain system security, and support ongoing quality improvement activities.
Whenever practical and permitted by law, Johns Hopkins Health Plans may use aggregated or de-identified information for analytics, reporting, operational improvement, research, quality assessment, and other lawful business purposes.
Mobile Device Permissions
To support certain Mobile Application features, Johns Hopkins Health Plans may request permission to access functions available on your mobile device, such as your camera, photo library, biometric authentication, location services, or push notifications. These permissions are requested only when necessary to support features you choose to use, such as uploading documents, scanning identification cards, enabling biometric sign-in, receiving notifications, or locating participating providers.
You may grant, deny, or later disable these permissions through your mobile device settings. If certain permissions are not granted or are later disabled, some Mobile Application features may not function as intended. -
Johns Hopkins Health Plans uses information collected through the Mobile Application to provide secure digital services, administer your health plan benefits, support your interactions with us, protect your information, improve the Mobile Application, and comply with applicable legal and regulatory requirements. The manner in which information is used depends on the features you choose to use, the services you request, your health plan coverage, and applicable legal requirements.
Providing Mobile Application Services
Johns Hopkins Health Plans uses your information to establish and maintain your Mobile Application account, verify your identity, authenticate your access, and provide the digital services available to you. Depending on your health plan and the features available through the Mobile Application, these services may include viewing benefit information, accessing digital member identification cards, reviewing claims and explanations of benefits, checking prior authorization status, locating participating providers, viewing pharmacy information, communicating securely with Johns Hopkins Health Plans, uploading documents, and accessing other digital tools made available to Members.
Information collected through the Mobile Application also allows Johns Hopkins Health Plans to personalize certain features, remember user preferences where appropriate, and provide a consistent and secure experience across supported devices.
Administering Your Health Plan
The Mobile Application serves as a secure extension of the services Johns Hopkins Health Plans provides to its Members. Information available through the Mobile Application may be used to verify eligibility, administer benefits, process claims, coordinate care, support care management activities, respond to Member inquiries, investigate issues affecting Member accounts, administer pharmacy benefits, and perform other activities necessary to operate your health plan.
The use of Protected Health Information for treatment, payment, and health care operations is governed by the Johns Hopkins Health Plans Notice of Privacy Practices and applicable law.
Communicating With You
Johns Hopkins Health Plans uses your information to communicate with you regarding your Mobile Application account, respond to questions, provide customer support, deliver secure messages, notify you about available Mobile Application features, and send communications relating to your health plan coverage and benefits.
Certain communications are necessary to administer your health plan or protect the security of your account. These communications may include password reset notifications, multi-factor authentication codes, security alerts, important benefit information, responses to your requests, and other operational communications. Because these communications support the administration of your health plan or the security of your account, they generally cannot be disabled.
Where available, you may choose whether to receive certain optional communications, such as Mobile Application notifications or other non-essential communications.
Protecting Your Account and Our Systems
Protecting Member information is a core responsibility of Johns Hopkins Health Plans. Information collected through the Mobile Application is used to verify user identities, authenticate access, monitor account activity, detect unauthorized access attempts, investigate suspicious activity, prevent fraud, maintain the confidentiality and integrity of Member information, and protect the Mobile Application and supporting systems from misuse or cybersecurity threats.
Johns Hopkins Health Plans also uses information to investigate potential violations of applicable agreements, respond to suspected security incidents, maintain business continuity, and improve the security of our digital services over time.
Improving Digital Services
Johns Hopkins Health Plans continually evaluates how Members use the Mobile Application to improve functionality, enhance usability, identify technical issues, develop new features, and better understand how digital services support Member needs.
Whenever practical and appropriate, Johns Hopkins Health Plans uses aggregated or de-identified information for analytics, reporting, operational planning, quality improvement, research, and other lawful business purposes. Aggregated and de-identified information is not used to identify individual Members.
Meeting Legal and Regulatory Requirements
Johns Hopkins Health Plans uses information collected through the Mobile Application to comply with applicable federal and state laws, regulations, accreditation standards, contractual obligations, court orders, lawful governmental requests, and other legal responsibilities.
Information may also be used to support audits, investigations, compliance activities, quality improvement initiatives, accreditation reviews, litigation, dispute resolution, records management, and other activities necessary to satisfy legal or operational obligations. -

Johns Hopkins Health Plans believes Members should understand the choices available to them when using the Mobile Application. While certain uses and disclosures of information are necessary to administer your health plan and therefore generally cannot be limited, you have meaningful choices regarding many aspects of your Mobile Application experience.
Managing Mobile Device Permissions
You may choose whether to allow the Mobile Application to access certain features of your mobile device, including your camera, photo library, biometric authentication, location services, or push notifications. These permissions may support specific Mobile Application features such as uploading documents, locating participating providers, enabling biometric sign-in, or receiving notifications.
You may grant, deny, or later disable these permissions through your device settings. If certain permissions are disabled, some Mobile Application features may not function as intended.
Communication Preferences
Where available, you may manage your preferences for receiving certain optional electronic communications. Johns Hopkins Health Plans may, however, continue to send communications necessary to administer your health plan, protect the security of your account, respond to your requests, provide important benefit information, or comply with legal or regulatory requirements.
Information Sharing Necessary to Administer Your Health Plan
Some uses and disclosures of information are required to administer your health plan and support the Mobile Application. For example, Johns Hopkins Health Plans must use and share information to verify eligibility, process claims, coordinate care, administer benefits, conduct health care operations, detect and prevent fraud, maintain system security, and comply with applicable legal requirements.
Because these activities are necessary to administer your health plan and fulfill legal obligations, they generally cannot be limited or opted out of.
Other uses of information may be subject to your preferences or rights under applicable law.
Requests to Limit Certain Uses or Disclosures
Depending on the circumstances and applicable law, you may request that Johns Hopkins Health Plans limit certain uses or disclosures of your Protected Health Information. Johns Hopkins Health Plans will review each request and respond in accordance with applicable legal requirements. Except where required by law, Johns Hopkins Health Plans is not required to agree to every requested restriction.
Additional information regarding your privacy rights is available in the Johns Hopkins Health Plans Notice of Privacy Practices.
State Privacy Rights
Depending on your state of residence, the type of information involved, and applicable law, you may have additional privacy rights regarding certain Personal Information collected through the Mobile Application. These rights may include the right to request access to, correction of, deletion of, or a copy of certain Personal Information, to appeal certain decisions regarding your privacy requests, or to exercise other rights provided under applicable law. Johns Hopkins Health Plans will review and respond to requests in accordance with applicable federal and state law. Certain state privacy rights may not apply to information governed by HIPAA or other applicable legal exemptions. Additional information regarding state-specific privacy rights, where applicable, may be available through Johns Hopkins Health Plans' general privacy notices or other supplemental privacy disclosures. -
You are responsible for maintaining accurate account information and promptly updating information that changes, including your contact information, when appropriate.
If you no longer wish to use the Mobile Application, you may discontinue using it at any time. Deleting the Mobile Application from your mobile device does not terminate your health plan coverage, close your Member account, revoke an Authorized Representative’s authority, or require Johns Hopkins Health Plans to delete information that must be retained under applicable law or Johns Hopkins Health Plans records retention policies.
If you believe your Mobile Application account has been accessed without authorization or your account credentials have been compromised, please notify Johns Hopkins Health Plans promptly using the Contact Us information below so appropriate measures may be taken to help protect your information.
Mobile Technologies
The Mobile Application uses technologies that help provide secure, reliable, and efficient digital services. These technologies support the operation of the Mobile Application, help protect your account, improve system performance, troubleshoot technical issues, and enhance your overall experience.
Depending on the features available through the Mobile Application, these technologies may include application programming interfaces (APIs), software development kits (SDKs), device identifiers, authentication technologies, encryption technologies, diagnostic tools, analytics technologies, and other technologies necessary to support Mobile Application functionality.
Johns Hopkins Health Plans selects and implements these technologies to support legitimate business, operational, security, and regulatory purposes. The Mobile Application is not designed to collect information unrelated to the services it provides.
Mobile Device Features
Certain Mobile Application features rely on functions available on your mobile device. For example, the Mobile Application may request permission to use your camera to upload documents or scan documents, access your photo library to attach images, enable biometric authentication for secure sign-in, use location services to help locate participating providers or other nearby resources, or send push notifications regarding account activity or information you choose to receive.
These permissions are requested only when necessary to support features you elect to use. You may grant, deny, or later disable these permissions through your mobile device settings. If certain permissions are denied or disabled, some Mobile Application features may be unavailable or may not function as intended.
Analytics and Performance
Johns Hopkins Health Plans evaluates how the Mobile Application performs to improve usability, identify technical issues, measure system performance, and enhance digital services over time. Information collected for these purposes may include technical information about your device, application performance, authentication activity, feature usage, and diagnostic information.
Whenever practical and appropriate, Johns Hopkins Health Plans uses aggregated or de-identified information for analytics, reporting, quality improvement, operational planning, research, and other lawful business purposes. Aggregated or de-identified information is not used to identify individual Members.
Security Technologies
The Mobile Application also uses technologies designed to protect your account and safeguard Member information. These technologies support identity verification, authentication, fraud detection, cybersecurity monitoring, encryption, system integrity, and other security measures intended to reduce the risk of unauthorized access or misuse.
Johns Hopkins Health Plans continually evaluates these technologies to address evolving cybersecurity threats, improve system resilience, and maintain the security of our digital services. -
For your convenience, the Mobile Application may provide links to third-party websites, applications, provider portals, pharmacies, telehealth services, wellness resources, or other digital services that are not owned or controlled by Johns Hopkins Health Plans.
This Privacy Notice applies only to the Johns Hopkins Health Plans Mobile Application. When you leave the Mobile Application or access a third-party website or service, the privacy practices, terms of use, and security practices of that organization apply.
Johns Hopkins Health Plans encourages Members to review the privacy notices and terms of use of any third-party websites or services they choose to access. Johns Hopkins Health Plans is not responsible for the privacy, security, availability, or content of third-party websites or services. -
Johns Hopkins Health Plans works with carefully selected third-party organizations that support the operation, maintenance, security, and functionality of the Mobile Application and assist in administering health plan services.
Depending on the services provided, third-party service providers may support cloud hosting, identity verification, cybersecurity, data storage, application development and maintenance, document management, customer communications, analytics, customer support, payment processing where applicable, and other operational functions.
When a third-party service provider receives access to Personal Information or Protected Health Information on behalf of Johns Hopkins Health Plans, including Business Associates where applicable, the organization must protect that information in accordance with applicable law, contractual obligations, and Johns Hopkins Health Plans privacy and security requirements.
Third-party service providers may use information only as necessary to perform authorized services for Johns Hopkins Health Plans and may not use or disclose information for their own independent business purposes except as permitted or required by law.
Johns Hopkins Health Plans periodically evaluates service providers through contractual oversight, security reviews, operational monitoring, or other appropriate governance activities to help ensure they continue to satisfy applicable privacy, security, operational, and contractual requirements. -
Protecting Member information is one of Johns Hopkins Health Plans' highest priorities. Johns Hopkins Health Plans maintains administrative, technical, and physical safeguards designed to protect the confidentiality, integrity, and availability of Personal Information and Protected Health Information maintained through the Mobile Application.
These safeguards are designed to protect information against unauthorized access, use, disclosure, alteration, or destruction and include measures supporting identity verification, authentication, encryption, access management, system monitoring, audit logging, cybersecurity detection and response, workforce training, and other information security practices consistent with applicable legal requirements and recognized industry security practices.
Johns Hopkins Health Plans regularly evaluates and enhances its privacy and security program to address changes in technology, evolving cybersecurity threats, regulatory requirements, and operational risks.
Although Johns Hopkins Health Plans works diligently to protect Member information, no method of transmitting or storing electronic information can be guaranteed to be completely secure. Members should also take reasonable steps to help protect their information by safeguarding account credentials, maintaining the security of their mobile devices, and promptly reporting suspected unauthorized access using the Contact Us information below.
Your Role in Protecting Your Information
Protecting your information is a shared responsibility.
Members can help protect their information by maintaining the confidentiality of usernames, passwords, authentication codes, and other account credentials; using security features available on their mobile devices, such as passcodes or biometric authentication; installing Mobile Application updates when available; and promptly notifying Johns Hopkins Health Plans if they believe their account credentials have been compromised or their mobile device has been lost, stolen, or accessed without authorization.
Johns Hopkins Health Plans will never ask you to disclose your password through an unsolicited email, text message, or telephone call. -
Johns Hopkins Health Plans is committed to making the Mobile Application and related communications accessible to all Members.
Members who need assistance using the Mobile Application or who require communications in an alternate format, auxiliary aids and services, or language assistance should contact Member Services using the phone number on their Member identification card. Johns Hopkins Health Plans provides accessible communications and language assistance in accordance with applicable federal and state laws and will make reasonable efforts to accommodate Members with disabilities and other communication needs.
Additional accessibility information may be available through the Mobile Application or the Johns Hopkins Health Plans website. -
Johns Hopkins Health Plans retains information collected through the Mobile Application only for as long as necessary to provide Mobile Application services, administer your health plan, satisfy applicable legal and regulatory requirements, support audits and investigations, resolve disputes, maintain appropriate business records, enforce applicable agreements, and fulfill other legitimate operational purposes.
Retention periods vary depending on the type of information involved, applicable federal and state laws, accreditation requirements, contractual obligations, litigation holds, and Johns Hopkins Health Plans records retention schedules.
When information is no longer required to be retained, Johns Hopkins Health Plans securely disposes of or de-identifies the information in accordance with applicable legal requirements and organizational policies. -
The Mobile Application is intended for use by Members and Authorized Representatives in connection with health plans administered by Johns Hopkins Health Plans. Members and Authorized Representatives may access the Mobile Application while traveling or temporarily located outside the United States, subject to applicable technical, operational, and legal limitations.
If you access the Mobile Application from outside the United States, information transmitted through the Mobile Application may be subject to the laws of the country from which you access the Mobile Application. Regardless of where you access the Mobile Application, Johns Hopkins Health Plans will protect your information in accordance with applicable United States laws, contractual obligations, and its privacy and security practices.
Access to the Mobile Application from outside the United States may be limited or unavailable due to geofencing, local legal requirements, technical limitations, cybersecurity protections, network availability, or other operational considerations. Certain features and functionality may not be available when the Mobile Application is accessed from outside the United States. -
Johns Hopkins Health Plans is committed to respecting the privacy rights of our Members and providing clear information about how Personal Information and Protected Health Information are collected, used, shared, protected, and retained. Depending on the information involved and applicable law, you may have certain rights regarding information maintained by Johns Hopkins Health Plans.
If information available through the Mobile Application constitutes Protected Health Information, your rights are described more fully in the Johns Hopkins Health Plans Notice of Privacy Practices. Those rights may include the right to request access to your Protected Health Information, request amendments to certain information, request an accounting of certain disclosures, request confidential communications, request restrictions on certain uses or disclosures where permitted by law, and obtain a copy of the Notice of Privacy Practices.
Depending on your state of residence and the type of information involved, you may also have additional rights under applicable state privacy laws. Johns Hopkins Health Plans will review and respond to requests in accordance with applicable legal requirements. Certain state privacy rights may not apply to information that is governed by HIPAA or other applicable legal exceptions.
To help protect your privacy and safeguard Member information, Johns Hopkins Health Plans may verify your identity or the authority of your Authorized Representative before processing requests relating to your information.
Johns Hopkins Health Plans will not discriminate against you for exercising any privacy rights available under applicable law. -
The Mobile Application is intended for use by enrolled Members of Johns Hopkins Health Plans and by properly Authorized Representatives acting on behalf of Members when permitted by applicable law.
When a Member is a minor or otherwise lacks legal authority to manage his or her own health information, access to the Mobile Application may be provided to a parent, legal guardian, personal representative, or other Authorized Representative whose identity and authority have been verified by Johns Hopkins Health Plans.
Johns Hopkins Health Plans does not knowingly permit unauthorized individuals to establish Mobile Application accounts or access Member information. If Johns Hopkins Health Plans becomes aware that an account has been created or accessed in a manner inconsistent with applicable law, this Privacy Notice, or Johns Hopkins Health Plans policies, access may be suspended, restricted, or terminated while the matter is reviewed. -
Johns Hopkins Health Plans may revise this Privacy Notice from time to time to reflect changes in applicable laws or regulations, Mobile Application functionality, privacy or security practices, operational processes, contractual requirements, or other business needs.
When material changes are made, the updated Privacy Notice will be made available through the Mobile Application and will include a revised Effective Date. When required by applicable law, Johns Hopkins Health Plans will provide additional notice regarding material changes. Your continued use of the Mobile Application following the Effective Date of an updated Privacy Notice constitutes acknowledgment of the revised Privacy Notice to the extent permitted by applicable law.
Johns Hopkins Health Plans encourages Members and Authorized Representatives to review this Privacy Notice periodically to remain informed about how information collected through the Mobile Application is protected. -

Submitting a privacy concern or complaint will not affect your eligibility for benefits, access to care, or the services provided by Johns Hopkins Health Plans.
