FAQs About Medical Records

The following information provides answers to common questions about obtaining copies of your medical records from the Johns Hopkins Hospital’s Health Information Management Department (HIM) (formerly called Medical Records).

May I receive a copy of my medical records by mail?

We can mail your medical records to the address you specify on the  Authorization form, or you can arrange to pick them up during the HIM Department’s hours of operation, Monday-Friday, 8am-5pm in the Phipps Building, Room B150.

Will you fax medical information in response to a request?

We will fax medical records only to another health care provider or facility for continuing medical care.

What are your hours of operation?

We are open Monday-Friday, from 8am-5pm and closed on national holidays.
