Adult psychiatric treatment at Bayview occurs in a variety of settings from emergency care to inpatient units, outpatient clinics, and community programs to support recovery. Building on a long tradition of serving the comprehensive needs of the community, Bayview Psychiatry also has a number of specialty areas including chronic mental illness, substance use disorders, geriatric psychiatry, neuropsychiatry (dementia, Alzheimer's disease and traumatic brain injury), sleep disorders, and behavioral medicine — the interface between psychiatry and other areas of medicine (e.g., burns, cardiovascular disease, chronic pain).

Adult Case Management

The Adult Case Management Program provides coordination and management of mental health services for individuals in the East Baltimore community who meet criteria defined by the Maryland Department of Health and Mental Hygiene. Case managers help clients who have a chronic and persistent mental illness access needed community services while encouraging them to live as independently as possible. The program provides home visits, entitlement procurement, assistance with housing, service coordination, help with medication, and wellness education. Services are currently available for Baltimore City residents and eastern Baltimore county residents. All clients must be authorized for service by Beacon Health Options, the company that provides administrative services for mental health care for the Medicaid program in Maryland.

Assisting adults who have psychiatric disabilities to reach their full potential

Creative Alternatives at the Johns Hopkins Bayview Medical Center is an innovative program that serves adults who have serious and persistent mental illnesses and who have not benefited from traditional services. The program combines mental health treatment, rehabilitative services and assistance with daily living. Program participants (called members) have the opportunity for community integration and personal growth.

Creative Alternatives works with members in hospitals, shelters, and in the community. On entering the program, members receive services from a team of professionals that includes a psychiatrist, nurse, case managers and community support assistants. A team member is available 24 hours a day in the event of a crisis or just to offer reassurance. Specialists in employment, housing, substance abuse, peer support and entitlements also support the members.

Creative Alternatives helps members with their medical and mental health needs in addition to assisting them in achieving their goals. These goals may involve:

Creative Alternatives is funded entirely by grants from Maryland's Behavioral Health Administration and Behavioral Health System Baltimore. Members do not have to pay to be a part of the program, nor do they have to forego their government benefits.

Intensive Outpatient Program for Adults

The Intensive Outpatient Program for Adults (IOPA) is part of the continuum of services offered in the Community Psychiatry Program (CPP).

This program is appropriate for:

• People who need more intensive treatment to divert inpatient hospitalization.
• People who need more intensive support upon discharge from inpatient hospitalization.
• People who are receiving outpatient treatment but experiencing worsening symptoms and a decline in functioning.
• People who would benefit from differential diagnosis evaluation.
• People who would benefit from intensive coping skills training in a group setting.

Services Offered

A team of licensed mental health professionals provide evidence-informed, short-term, intensive psychiatric treatment for patients who are able to function safely while they continue to live in the community. Patients attend the program three days a week from 9:30am-12:30pm. A typical treatment day consists of three groups for symptom reduction and skills training. The groups are influenced by CBT (Cognitive Behavioral Therapy), DBT (Dialectical Behavior Therapy), and Mindfulness with an emphasis on skills training. Patients also receive weekly individual psychotherapy sessions with an IOPA therapist and weekly medication reviews by the IOPA psychiatrist. Transportation assistance may be provided to and from the program for those residing in a limited area. When discharged, most patients return to their former outpatient therapist, empowered with the skills to take part in their lives more fully.

Feedback Informed Treatment

On average, people who complete IOPA tend to demonstrate reductions in severity of symptoms, including depression, anxiety, and stress (as measured by the Depression Anxiety Stress Scale-21). Moreover, they report an increase in use of healthy coping behaviors, and a decrease in use of unhealthy behaviors (as measured by the DBT Ways of Coping Checklist).

Adult Mobile Treatment

The Mobile Treatment Program serves adults in the community with serious and persistent mental illness who have difficulty following through with traditional outpatient services. In addition to being unable to engage in traditional outpatient treatment, an individual must have a history of inpatient psychiatric treatment, extensive usage of emergency services, and/or arrests associated with mental illness.

Patients receive psychiatric services in their home from a team of psychiatric professionals. By providing consistent services in the community, patients achieve greater treatment compliance and experience fewer hospitalizations. The goal is to help clients toward recovery so that they can return to traditional forms of psychiatric treatment in the community.

Adult Psychiatric Rehabilitation

The Adult Psychiatric Rehabilitation Program (Adult PRP) is a Center of Excellence in Recovery. The Adult PRP provides a full range of psychiatric rehabilitation services for individuals with severe and persistent mental illness. Services Provided

Intensive On-site Services (IOS) provides psychosocial educational services, which support individuals in achieving their identified life goals and tools to live well. All educational services are defined by principles of recovery.

IOS offers care Monday through Friday, from 8:30 a.m. - 1 p.m.

• Psychosocial Education Group Services: IOS provides a number of psychosocial education groups based on the mental health recovery and wellness model. These groups support individuals in achieving their mental health recovery goals by educating them on tools and strategies to maintain wellness, and help them work toward improving their overall quality of life.
• Individual Services: Each client is paired with a rehabilitation therapist who can offer assistance with navigating the entitlement process; coordinating medical and psychiatric provider appointments; connecting to community resources and crisis response; and assessing transportation needs.

Supported Housing Services
Clients work with Adult PRP staff to obtain their desired living arrangement. Resources also are available for those who require assistance in securing and managing independent living, supervised living or transitional housing.

Supported Employment Services
Services include pre-employment training and assessment; job development and placement; job coaching; benefits management; vocational training and counseling; and ongoing support services.

Pre-Employment Transitional Services
For individuals 14-22, services include Job Exploration Counseling and Work-Based Learning Experience
