Applicants are asked to submit the following items to the GIM Fellowship Coordinator:

• Completed application
• GIM Fellowship Track Selection and Area of Interest form
• Curriculum vitae
• Copies of college, medical school and any advanced degree (e.g., master's degree) program transcripts (Please note that copies of these are sufficient; original transcripts are not required.)
• One-page personal statement of research interests
• Three letters of recommendation (These should be sent by the authors directly to the GIM Fellowship Coordinator.)
