At the Johns Hopkins Healthful Eating, Activity & Weight Program (HEAWP), our goal is to be a resource that provides a patient-centered experience for the delivery of multidisciplinary, evidence-based clinical services to prevent or manage chronic diseases through lifestyle changes and weight loss for your patients. We are committed to providing services supported by robust scientific evidence.

We believe in a collaborative patient management approach with referring physicians. Our physicians do not assume the primary care of your patients.

Referrals

Conditions for which patients may be referred to HEAWP may include, but are not limited to:

  • Overweight/Obesity
  • Diabetes & Prediabetes
  • Heart Disease
  • Hypertension
  • Metabolic Dysfunction-Associated Steatotic Liver Disease
  • Women’s Health Conditions including PCOS, Menopausal Weight Gain, Infertility, Lipedema, and Breast Cancer Survivors
  • Perioperative Care for Bariatric Procedures

How to Refer

For providers within Johns Hopkins
Place a referral in Epic using the “Referral to Internal Medicine” order, then select the button for the Healthful Eating, Activity & Weight Program and the indication for referral. Patients should call 410-616-7420 to schedule an appointment.

For providers outside Johns Hopkins
Please fill out the Physician Referral Form and fax to the Johns Hopkins Healthful Eating, Activity & Weight Program at 443-367-3215. If the patient’s insurance company also requires a specific referral form, please fax that as well. Patients should call 410-616-7420 to schedule an appointment.
