Case Management for Women and Infants
We walk alongside mothers during pregnancy and after birth, offering guidance, encouragement, and access to vital resources. For little ones from birth up to age 2, we help families ensure healthy growth, development, and strong starts in life
Our case management program offers guidance and support for:
- Prenatal & Postpartum Women: Individualized care plans, ongoing check-ins, and access to vital health and social resources.
- Infants (Birth to Age 2): Monitoring development, connecting families to pediatric resources, and ensuring a healthy start to life.
Enrollment Criteria:
- Pregnant women (any stage of pregnancy)
- Postpartum women (up to 6 months after delivery)
- Infants and children up to age 1 (however we will serve until the child reaches age 2)
- Families seeking support with health, social services, or parenting resources
