Appointments & Referrals
The Johns Hopkins Melanoma Program includes faculty members in a variety of specialties. Appointments for patients already diagnosed with melanoma can be made through a single-access point to facilitate both physician and self referrals.

I have a diagnosed melanoma…
Call 410-616-7660 to make an appointment with an expert in the Melanoma Program. A downloadable Appointment Request Form is available and can be faxed to (410) 616-7663.
Information that will be needed to make an appointment includes:
- Patient’s age, contact information and brief medical history
- Pathology reports
- MRI, CT, PET and lab results (if these tests have been done)
- Operative notes, if any
- Copy of insurance card(s)
For your first appointment at the Johns Hopkins Melanoma Program, you will need to bring your primary care provider referral (if needed) and copies of any scans/X-rays (MRI, CT, PET).
We also need to have our pathologists review your pathology slides that showed melanoma. We will discuss with you how they can be obtained.
