After the appointment request form is submitted, your care coordinator will email you to learn more about your current medical condition, any treatments you are receiving, and what your Johns Hopkins treatment goals are. The more complete your responses, the faster we can connect you with the right specialists.

To complete your appointment request, we’ll need a clear copy of your passport and a copy of your medical insurance card (if you plan to use insurance).
Visa Information
Johns Hopkins Medicine is in the United States. If you do not have a U.S. visa, please contact your local U.S. embassy or consulate to inquire about processing times and requirements. Once your appointment is scheduled, we will provide an official appointment confirmation letter, which may support your visa application. (Unfortunately, we cannot provide visa invitation letters.)
