Community Health Needs Assessment

Our hospital uses the Community Health Improvement Guiding Principles proposed by the Johns Hopkins Medicine Community Benefits Advisory Council to help guide our collective commitment and efforts. They are as follows:

  • Partnering with those community leaders and community-based organizations that serve as stewards to guide and educate JHM to best serve the needs of each population
  • Integrating the interests, assets and expertise of our health system, academic institutions and the community to reach mutually agreed-upon outcomes
  • Developing transparent, coherent approaches to plan, implement, evaluate and disseminate sustainable health improvement programs and policy interventions

In fiscal year 2025, in conjunction with Howard University Hospital, our hospital conducted an extensive community health needs assessment (CHNA) to identify the most important health issues in our community.

As a result, we identified the following health priorities:

  • Health Conditions (including mental health, chronic diseases, substance use, and maternal/infant health)
  • Healthcare Access (with a focus on system navigation)
  • Social Determinants of Health (including housing, safety, food security, and economic inequality)


Our hospital’s commitment to improving the health and well-being of our community is demonstrated through the deliberate planning of community benefit activities in accordance with the Community Health Improvement Plan (CHIP).

Sibley Memorial Hospital – in partnership with Howard University Hospital – is proud to release our 2025 Community Health Needs Assessment (CHNA). This work is undertaken in partnership, is data-driven, and engages the community. The ultimate pursuit is an equitable and sustainable state of health for District of Columbia residents.

Community Health Improvement Plan

Our hospital will soon post the Community Health Improvement Plan. The implementation plan connects the hospital, community partners, clinicians and other resources with identified health needs. Our hospital aligns health priorities with the areas of greatest identified need and considers where the hospital’s resources will generate the greatest impact.
