Make an Appointment
For new patients looking to book an appointment the best way is to contact our scheduling team by sending an email at ([email protected]). Team member will respond back via email. Four items are helpful for the visit. These are as below:
- Referring physician /provider notes: You may request their office to fax the notes to us at 410-502-4133.
- MRI, PET scan imaging: If you had the imaging done at an outside Hopkins facility, you may either request them to send the images electronically to Johns Hopkins AMBRA portal or obtain the CDs and bring it with you during the visit.
- FTD questionnaire: Complete FTD questionnaire.
Prepare for your Appointment
Prior to your schedule appointment, you will receive the following information in the mail:
- A letter confirming your appointment
- New patient questionnaires
- Family history and medical history forms
- Other helpful information, such as maps and directions
Please fill out the questionnaires and send them prior to your appointment. We also request that you bring:
- Your insurance card
- Any medical records that have not been forwarded to the clinic beforehand
Our physicians may request testing such as blood work, electroencephalogram (EEG), brain imaging, or cerebrospinal fluid (CSF) analysis (i.e., a lumbar puncture, LP, or “spinal tap”) prior to the evaluation or follow-up visit. If this is the case, specific instructions will be provided to you before the appointment. Please call us at 410-955-5147 if any questions arise in obtaining the requested tests.

The location is 600 N. Wolfe Street, Meyer Building, 2nd Floor, Suite 235 (Off of the main loop at McElderry and Wolfe).
Patients should park at Orleans Street garage or use the Valet Parking Service at the main entrance to the hospital at 1800 Orleans Street. From the Orleans Garage, patients should take the garage elevator to the 4th floor to access the pedestrian bridge to the main hospital and an information desk where they can be directed to the Meyer building. There are wheelchairs available for use at the main entrance information desks.
Patients can also be dropped off at the lobby entrance at 600 North Wolfe Street, which is the Wolfe Street main loop (the corner of Wolfe and McElderry Streets prior to parking in the Orleans Garage.If a patient is being dropped off who requires assistance, it is advisable to bring an assistant to stay with the patient at the lobby entrance while the car is being parked in the Orleans Garage. There is an information desk at this entrance, too, as well as wheelchairs available and the Meyer Building is a short distance from this entrance. -
AFTD-affiliated Online Care Partners Support Group
The group is open to anyone who would benefit from a safe, supportive space to connect with others living with FTD, to ask questions and learn practical strategies to navigate day-to-day challenges. Meetings are on the second Wednesday of every month at 4 pm via zoom. Email [email protected] to learn more, register and receive a zoom link.
Refer a Patient
To refer a patient, our team needs the following information:
- The patient’s contact information (name, address and phone numbers).
- Insurance information.
- Pertinent medical records, including clinic notes, lab results, neuropsychological test reports, and brain imaging reports and films, preferably on computer disc.
We will contact your patient once we have received his or her medical records and they have been reviewed by one of our team members. This process typically takes no more than a week. You may submit medical records by mail or fax to Mary Ann Wylie, above.
