Occupational Therapy Fieldwork Education at Johns Hopkins Bayview

Welcome to the Occupational Therapy Department at the Johns Hopkins Bayview Medical Center. We look forward to your clinical affiliation. We contract with various schools throughout the country.

This site contains the necessary information you will need in order to begin your fieldwork experience, including the required paperwork you’ll need to complete, information about the Rehabilitation Department, parking fees and contact information. Please note that housing options are available upon request; however the hospital does not endorse any housing facility. This information is provided for informational purposes only.

For Level 1 Students

Documentation of a flu shot and COVID-19 vaccine(s) are needed three weeks prior to the internship.

Please complete the demographic form and email it to occupational health.

For Level 2 Students

Below is a list of the required paperwork that should be submitted at least three weeks in advance of the start of your internship. After completing, please scan and e-mail to Adersh O’Day, student coordinator, at [email protected].

  1. New User ID Request Form (.pdf)
  2. Confidentiality Form (.pdf)
  3. Please also review the OT/PT Student Orientation Guide (.pdf) and sign the last page. Students should return pages 34-36 to their supervisor by the end of the first week.
  4. Copy of your immunization records:
  5. Copy of 2-step PPD testing within the last year or a blood test within the last 6 months.
  6. Copy of a 10 panel drug test within the last 3 months. Contact occupational health at 410-550-0477 for more information.
  7. Copy of a current CPR certification (within two years)
  8. A completed criminal background check should be emailed to the following:

    Please let them know you are an upcoming OT student. The following screens should be included:

    2. Residency History
    3. Social Security Verification
    4. Nationwide Sexual Offender Index
    5. Nationwide Healthcare Fraud and Abuse Scan
