Financial Information
Referrals or Authorizations for Appointments
Your insurance provider may require a doctor referral or authorization prior to your audiology appointment. It is your responsibility to acquire these referrals or authorizations prior to your appointment, otherwise your insurer may not cover the cost of your appointment.
After your first appointment, the department’s billing staff will handle all additional paperwork required by your insurance company.
Payments and Insurance
Your deductible and/or copay is expected at the time of service. You can receive a copy of your copayment schedule from your insurance company. You will receive a bill by mail for any amount not covered by your insurance.
If you are not using insurance, you will be responsible for the full cost of care at the time of your appointment.
A financial counselor is available to assist you with any billing concerns or questions. There are also organizations that may provide financial assistance for hearing aids.
