Maryland Family Physician Summary of the Maryland Primary Care Program
MDPCP supports the delivery of advanced primary care throughout the state and allows community providers to play an increasingly important role in improving health outcomes and controlling total health care spending growth.
This links to a set of resources for clinicians developed with the Division of Geriatrics and others. These provide education, training, and staff certification programs designed to improve the quality of care for patients over the age of 65.
The purpose of the JHGWEP is to develop an integrated geriatric primary care health care workforce that maximizes patient and family engagement and results in improved health outcomes in Maryland. We propose to accomplish this by integrating geriatric principles into the workflow of selected community-based primary care practices, and by engaging older adults in their own health care by integrating and expanding the services of community-based health advocacy and educational organizations.
