Geriatric Care Education & Medical Homes
This links to a set of resources for clinicians developed with the Division of Geriatrics and others. These provide education, training, and staff certification programs designed to improve the quality of care for patients over the age of 65.
Geriatric Workforce Enhancement Program (GWEP)
The purpose of the JHGWEP is to develop an integrated geriatric primary care health care workforce that maximizes patient and family engagement and results in improved health outcomes in Maryland. We propose to accomplish this by integrating geriatric principles into the workflow of selected community-based primary care practices, and by engaging older adults in their own health care by integrating and expanding the services of community-based health advocacy and educational organizations.
