Refer a Patient

To refer a patient for neurofibromatosis or schwannomatosis, contact the on-call physician, the on-call neurologist or neurosurgeon at the Hopkins Access Line (HAL) at 410-955-9444. The Hopkins Access Line accepts physician-to-physician referrals only.

To make the referral process as efficient as possible, please fax the following information and medical records when referring your patient to the Johns Hopkins Comprehensive Neurofibromatosis Center. 

  • Patient name
  • Age
  • Date of birth
  • Contact information
  • Diagnosis
  • Reason for referral 

International patients may be referred through Johns Hopkins International. Patient admission will be directed to The Johns Hopkins Hospital or the Johns Hopkins Bayview Medical Center depending on the sub-specialty care required.
