With roots in providing primary care to patients who suffer from chronic medical illnesses including HIV, the Comprehensive Care Practice has evolved to address rising priorities in the community. Our interdisciplinary and patient-centric care team is growing, and features a team of experts in primary care, as well as specialized care for patients with HIV, Hepatitis-C or substance use disorders.

  • Leadership and Staff

    Our staff includes clinical providers, case managers, peer recovery coaches, community health workers, patient service coordinators and medical assistants.

  • Appointments and Insurance

    Before you schedule an appointment, please contact your insurance company to make sure that the Comprehensive Care Practice and our providers are covered by your plan.

  • Effective, compassionate primary medical care from providers who treat their patients with respect and without judgment
  • Ongoing support to manage chronic illnesses
  • Buprenorphine treatment (this service is offered only to patients followed by one of CCP’s providers for their regular primary care)
  • Access to community resources
  • Referrals for specialty services
  • On-site case management and education services as well as lab services in our office.
