Pediatric Solid Organ Transplant Transition Program

The Johns Hopkins Pediatric Solid Organ Transplant Transition Program provides continuous care from childhood to young adulthood. Our goal is to give patients the necessary information and skills needed to manage their care by age 22, as recommended by the Organ Procurement and Transplant Network.

The transition program consists of six modules, which will be reviewed at clinic appointments. The goals of each module are:

  • Transplant knowledge
  • Medication knowledge
  • Adherence to medication and lab work
  • Understanding risk-taking behaviors
  • How to stay healthy
  • Self-advocacy
  • Reproductive health
  • Managing school and/or work
  • Self-esteem
  • Health insurance
  • Advanced directives
  • Identifying a support system

When it’s time to transfer to adult-focused care, research shows that success largely depends on patients taking ownership of their own medical issues. Our pediatric transplant programs work in close connection to our adult transplant programs to allow for a seamless transition to the adult care team through the Johns Hopkins Comprehensive Transplant Center.
