Pediatric Complex Care Collaborative Program

The Pediatric Complex Care Collaborative Program (PC3) is a consultative service that cares for children with chronic complex conditions who rely on medical technology. As a strictly consultative program, it does not replace your primary care physician.

Our Mission

The Johns Hopkins Pediatric Complex Care Collaborative is committed to improving care for our most medically complex patients and their families. We aim to enhance quality of life by providing innovative, compassionate care that ensures families feel understood and fully supported.

Our goal is to improve access to care through coordinated, comprehensive services that address health inequities and meet families where they are. By focusing on the unique needs and goals of each patient and family, we work to foster resilience and empower those we serve.

What care does PC3 offer?

Our program strives to provide wraparound services to our most complex patients and their caregivers, both inside and outside of the hospital.

Our team is available from 8 a.m. to 4 p.m., Monday through Friday to provide these services:

Inpatient consultation to help with continuity of care, long-term planning and transitions of care. This takes place at Johns Hopkins Children's Center.

Post-discharge follow-up visits to confirm safe discharge and identify any post-discharge needs.
Outpatient clinic appointments (2 to 4 times a year) to review coordinated care plans and identify unmet care and care coordination needs . This takes place in the David Rubenstein Child Health Building.
Enhanced care coordination from our medical and care management teams (including team meetings/discussions, documented multidisciplinary care plans and coordination of appointments when possible).

Telemedicine is available when an in-person appointment is not feasible.
