The Pediatric Complex Care Collaborative Program (PC3) is a consultative service that cares for children with chronic complex conditions who rely on medical technology. As a strictly consultative program, it does not replace your primary care physician.

Our Mission
The Johns Hopkins Pediatric Complex Care Collaborative is committed to improving care for our most medically complex patients and their families. We aim to enhance quality of life by providing innovative, compassionate care that ensures families feel understood and fully supported.
Our goal is to improve access to care through coordinated, comprehensive services that address health inequities and meet families where they are. By focusing on the unique needs and goals of each patient and family, we work to foster resilience and empower those we serve.
What care does PC3 offer?
Our program strives to provide wraparound services to our most complex patients and their caregivers, both inside and outside of the hospital.
Our team is available from 8 a.m. to 4 p.m., Monday through Friday to provide these services:
Patient Referrals
To refer a patient, please contact 410-502-6531. We will need to following information:
- Name
- Date of birth
- Diagnosis
- Medical technology used
- Referral contact number
After we review criteria, you will be contacted within two weeks of the initial referral with follow-up information.
Johns Hopkins providers can place an order for Pediatric Complex Care Management Evaluation in Epic. If accepted, we will notify you when the patient is scheduled to see a member of our team in clinic.
