Before starting your bariatric surgery journey at Sibley Memorial Hospital, we ask you to attend an information session to get familiar with the process. This can be done in two formats: via Zoom or by watching a recorded video.

After completing this form, you will receive instructions based on the format you choose. Attending a live Zoom session will give you a chance to meet our experts and ask questions. Upcoming online sessions are at noon ET on the dates below:

  • 2026 Sessions with Dr. Parker: January 15, February 19, March 19, April 16, May 21, June 18, July 16, August 20, November 19

Next Steps and What to Expect

  • Health Questionnaire: After you attend an information session, you will receive a questionnaire by email that includes questions about your health history.
  • Consultation: Once you send the questionnaire back and our team reviews it, we will contact you to schedule a consultation.
  • Insurance Coverage: Our team will send you a separate questionnaire to gather the necessary information to help you work with your insurer to determine coverage.
