An ASHP-accredited residency certificate will be awarded upon the successful completion of all residency requirements, including:

• Successfully completing all rotations and weekend/holiday service requirements and the corresponding evaluation(s)
• Completing, presenting and writing up a residency project
• Creating and presenting a poster at either the ASHP Midyear Clinical Meeting or another professional forum
• Presenting a residency project at the Eastern States Conference for Pharmacy Residents and Preceptors
• Completing, presenting and writing up a medication use evaluation
• Completing, presenting and writing up a Pharmacy and Therapeutics Committee monograph
• Completing, writing and publishing a departmental newsletter
• Presenting at least one Journal Club article
• Completing and presenting a formal seminar
• Abiding by the policies and procedures of the hospital and the department
• Completing and maintaining ACLS and BLS certification
