Practicum students must submit the following paperwork following the timeline(s) above:
- Attestation Form – Pre-Licensure Students OR Attestation Form – Graduate Students
- Waiver Release
- Confidentiality Statement
- JHED Request Form
- Preceptor packet (provided by the student to JHHCMC)
Students and schools will receive paperwork and instructions for submission following JHHCMC confirmation of practicum placement. Completed paperwork must be submitted following the timelines above.

All nursing students seeking a practicum placement at JHHCMC must submit a request through the Exxat One platform. Beginning 10/16/25, all student placements and onboarding will be managed exclusively through Exxat One. You can search for availability and request placements directly within the platform. Visit the Exxat One Help Center for detailed guides and resources on managing placements and onboarding. Please note, APRN students must identify their own preceptor. Also, please note, JHHCMC may not be able to place all requests.
