Following hospital discharge, some patients require continued regimens of intravenous (IV) antibiotics, which are administered outside of the hospital setting either in skilled nursing facilities or most frequently in the home. Our research informs evidence-based practices for patients, caregivers, and healthcare workers that improve treatment outcomes and has resulted in a decrease in hospital readmission.
