To arrange evaluations and request appointments, patients can call 410-955-9270 to speak with an agent who can begin the scheduling process. The following information will need to be faxed to 410-630-7504:

Laboratory test results
MRI reports (please do not send original films when requesting an appointment)
Referring physician and clinic notes
Copy of insurance card, front and back

We strongly suggest that patients contact their insurance company to make sure they can be seen at Johns Hopkins. Once we receive all completed forms and records, we will contact the patient to coordinate appointment(s) and arrange for forwarding any original films if necessary.
