Request an Appointment
New patients must be referred by a physician unless you have already received a neuromuscular diagnosis.
Before scheduling an appointment, you will need to provide the following:
- Contact information (name, address, and phone numbers)
- A copy of the front and back of your medical insurance card(s)
- Medical records pertaining to your neuromuscular diagnosis or symptoms, such as physicians’ office visit notes, a physician’s letter of referral, lab results, reports from EMG, MRI, CT or other tests.
- Please do not send radiology films – bring them with you to your appointment.
Once we have received your medical records, they will be reviewed by one of our physicians and our staff will contact you.
You or your referring physician may submit medical records by mail or fax to:
