Summary
The Glinkage study seeks to describe linkage to care services for people living with chronic conditions and released from jails and prisons throughout the United States. Chronic conditions of specific interest include HIV, substance use disorder, mental illnesses, diabetes, and hypertension. In addition to describing service provision, we are identifying barriers to care continuity that are not addressed by current services. This project uses semi-structured interviews of staff involved in program delivery (correctional services, health departments, community-based organizations) and people living with a chronic condition with recent experience of correctional facility release.
