Anticoagulation (AC) Management Service

The Johns Hopkins Anticoagulation (AC) Management Service is a team of expert physicians and pharmacists who specialize in the care of patients being treated with anticoagulation medications (also known as blood thinners). Patients may be prescribed blood thinners for treatment or prevention of blood clots. Our clinics are also often referred to as "Coumadin clinic" or "Warfarin clinic."

Hemostatic & Antithrombotic Stewardship Program

Patients with bleeding and clotting disorders often require medications to prevent severe complications. However, these medications can be high-risk if not administered by an expert team and are often associated with high cost. The mission of the Johns Hopkins Hemostatic & Antithrombotic Stewardship Program is to provide the highest quality of care to patients with bleeding and clotting disorders while reducing unwanted costs.

Our Johns Hopkins Hemostatic & Antithrombotic Stewardship Program is a unique collaboration between the Division of Hematology and Department of Pharmacy. Our team runs a factor approval service, develops evidence-based institutional guidelines for the proper use of bleeding/clotting agents, and provides real-time advice to our Hematology Inpatient team.
