Throughout your care journey, our dedicated nurse coordinator serves as your primary point of contact, helping navigate appointments, answering questions and connecting you with appropriate resources. We understand that aortic conditions affect not just patients but entire families, and we are committed to providing comprehensive support every step of the way.

Appointment Scheduling

Although a physician referral letter is not required to schedule an appointment, patients must have been referred to see a cardiac or vascular surgeon within the past 12 months. When you call, the scheduling agent will ask for your diagnosis and the name of your referring doctor.

You will also be asked to fax or upload your medical records and recent heart imaging studies. The doctor who diagnosed you can help gather these records. In some cases, our team may need to review the records before we can schedule your appointment.

Initial Consultation

During your first visit, you will meet with team members who will review your medical history and previous imaging, discuss family history of heart conditions and perform a thorough physical examination. We will take time to understand your symptoms, concerns and health goals to get a complete picture of your condition.

Diagnostic Evaluation

If additional testing is needed, we will coordinate advanced imaging studies and other tests, such as lab work. Our team will explain each test and its purpose in developing your personalized care plan.

For patients with inherited aortic conditions, our genetic counselors can provide guidance on family screening, inheritance patterns and the implications of genetic findings for patients and their relatives.

Treatment Planning

Our team will discuss your case in our multispecialty aortic conference, where experts from different fields work together to determine the optimal care approach. You will then meet with your care team to review recommendations, discuss options and address questions.

We are also committed to coordinating care with your referring doctor. Our team will share relevant consultation notes and treatment plans with your doctor throughout the process.

Procedure and Hospital Stay

If you need a procedure, our team will guide you through preparation, surgery and recovery. We provide comprehensive educational materials about aortic conditions, treatment options and what to expect before, during and after procedures.

Our specialized nursing staff and care coordinators are available to answer questions and ensure a smooth hospital experience.

Follow-Up and Long-Term Care

Once you are home after the procedure, you will have follow-up visits to monitor your recovery and aortic health. Our team will tailor the follow-up care plan to your needs and recovery progress, and will remain available if you have questions or new health concerns.

Our cardiac rehabilitation specialists and nutritionists help patients develop safe exercise programs and heart-healthy diet plans that support overall cardiovascular health and recovery.
