Community Health at Suburban Hospital

Suburban Hospital has long provided free diagnostic, outpatient, and inpatient health services to Mobile Medical Care, Inc, a local clinic that provides free to low-cost medical care for the uninsured. Moreover, Suburban provides these same free services and more to additional Montgomery Cares clinics located throughout Montgomery County. Montgomery Cares is a public-private partnership composed of 11 independent safety-net primary care clinics, six hospitals, and the Montgomery County Department of Health and Human Services.

In Montgomery County and throughout the State of Maryland, the need for specialty care is substantial. Subsidized specialty care programs in the area are saturated with long waiting lists. As a result, many county residents suffer complications from their diseases because they are unable to access timely specialty care. This inequality and inequity provided the impetus for the MobileMed Heart Clinic at Suburban Hospital, which strives to address a leading cause of death in Montgomery County — cardiovascular disease.

The MobileMed Heart Clinic at Suburban Hospital serves as a catalyst for addressing other county health priorities in similar manner. The clinic’s best-practice model draws upon the expertise and resources of its partners — Mobile Medical Care and Suburban Hospital — to offer one-of-a-kind, comprehensive specialty care services to low-income, uninsured residents of Montgomery County.

Suburban Hospital is committed to eliminating disparities associated with colorectal cancer. Suburban focuses outreach efforts on identifying individuals that may be at high risk for colorectal cancer and connecting them to available free screening programs through the Montgomery County Cancer Crusade.

Eleni Antzoulatos is the Health and Wellness Operations Supervisor for Suburban Hospital's Community Health & Wellness Department. As a public health professional with more than 15 years of experience, Eleni specializes on implementing healthy lifestyle programs for local businesses and the surrounding community. Eleni is a service minded individual dedicated to bringing high quality fitness, mind and body, nutrition, and health and safety learning opportunities to Montgomery County residents via webinars, screenings, and classes. Eleni is committed to creating a space for people to receive cutting edge health information via the Annual Women's and Men's Health Symposiums, and thoughtfully curating the content for the biweekly electronic WellWorks' Community Health & Wellness Calendar. Eleni is incredibly motivated to use her data analytic skills to manage the hospital's annual Community Benefit Report and leading the publication of the hospital's annual Community Health Improvement report. As an active member of the Greater Bethesda Chamber of Commerce, the Hospital Behavioral Health Workgroup and an alumni member of the Montgomery County Food Council, Eleni strives to provide constant insight on health improvement initiatives that address health equity and access.

Eleni holds a Bachelors of Arts in Biology from McDaniel College and Masters of Public Health from the George Washington University.

Emily is a Certified Health Education Specialist and manages the community health and wellness initiatives in southern Maryland. In collaboration with community partners, she leads efforts to improve outcomes in cardiovascular health and support behavior change among Maryland residents through programs that integrate physical activity, nutrition and social connection.

Emily’s career has primarily focused on leading program strategy, design and development of health education and promotion programs for children, youth and families for global nonprofit organizations, health systems and community-based organizations. She holds a Bachelor’s degree in Psychology from the University of Oklahoma, a Master’s degree in Public Health from the University of North Texas Health Sciences Center and an executive certificate in Social Impact Strategy from the University of Pennsylvania.

Kate McGrail manages the integration of community health improvement to support a culture that delivers high quality healthcare with improved outcomes and efficient utilization of resources. Specifically, Kate is a member of the Population Health Care Delivery team at Suburban Hospital, supporting population health operational strategies to improve performance on key utilization and cost goals.

Kate also leads efforts to reduce cardiovascular health disparities for the Hospital's community benefit service area as well as in southern Maryland, focusing on populations with higher rates of chronic health conditions such as hypertension and diabetes. In this capacity, Kate coordinates the planning, organization, development, and implementation of community education programs, health partnerships, and blood pressure and cholesterol screenings. To support Montgomery County's growing aging population, Kate coordinates the Village Ambassador Alliance, a collective created by Suburban Hospital to bridge the village-hospital connection through education, training, and resource allocation to support those older adults who wish to age in their own homes.

She holds a bachelor's degree in Economics-Accounting and Visual Arts from the College of the Holy Cross and a Masters of Public Health degree from the George Washington University. Kate is a certified public accountant and volunteer Treasurer and Board Member of the Maryland Public Health Association. In her spare time, Kate is a fitness instructor at barre3, where she incorporates her passion for well-being, movement, and music to motivate clients to find freedom in their bodies while improving functional strength and mindfulness.

Monique leads Suburban Hospital’s health transformation and improvement commitment to the community, which aims to improve the quality of life and reduce hospitalizations. Aligning hospital and community assets as well as focusing on unaddressed identified needs, she links health partnerships, community initiatives, strategic affiliations and collaboration with health coalitions, outreach activities, wellness programs and corporate projects to address established health barriers, disparities and inequities.

Monique serves on the boards of the Bethesda-Chevy Chase YMCA, Nexus Montgomery, the Healthy Montgomery steering committee and the advisory council board for the Archdiocesan Health Care Network. She is a graduate of the American Hospital Association and the Health Forum’s Leadership Creating Healthier Communities Fellowship, as well as a 2013 graduate of Leadership Montgomery. An 18-year member of the Bethesda-Chevy Chase Rotary Club, she has served for multiple years on the Board of Directors. As a member of the United States Peace Corps, Monique also volunteered as a Health Extensionist in Ecuador.

Monique completed her undergraduate degree at Washington College, Master's degree at Middlebury College and Masters of Business Administration at the Johns Hopkins Carey Business School.

Toney coordinates Suburban Hospital’s community health programs, enhancing access to care, promoting healthy behaviors, and addressing health inequities. He oversees local health initiatives, ensuring alignment with the hospital’s strategic goals through effective implementation, evaluation, and prioritization.

A strong collaborator, Toney fosters partnerships such as the Adopt-A-Family program, connecting the hospital with local schools to support families in need. He also organizes Senior Shape, a functional fitness program that enrolls close to 500 adults 55+ to help improve strength, balance, and reduce isolation.

His public health focus includes coordinating blood drives, wellness activities, events to support health advocacy, and implementation of cancer awareness and prevention initiatives in partnership with the department of health. Toney co-leads the Medical Exploring Program, which gives high school students hands on experience to discover the multiple facets of careers in healthcare and science.

Toney holds a Bachelor of Science in Community Health from the University of Maryland, specializing in Health Risk Behavior, and a Bachelor of Arts in Business Administration from Saint Leo University. He is a member of the American Public Health Association (APHA) and the National Society of Leadership and Success (NSLS). With over 20 years of distinguished military service, Toney brings strong leadership and logistics expertise to the health sector. His unique combination of skills, public service commitment, and military background drives a strategic, results-oriented approach to improving community health outcomes.

HeartWell is a free health education program designed for seniors to increase awareness about preventative guidelines, symptom recognition and management, medications, blood pressure monitoring, diet and lifestyle enhancements.
