Release of Medical Records
Patient records at Suburban Hospital are confidential. However, patients or their representatives with legal medical power of attorney can authorize the release of confidential patient information by completing a signed Release Form Release Form and returning it to our medical records office by fax, mail, or in person. The form should be completed and dated. Please note there is a fee for copying medical records.
Medical Records staff are available Monday through Friday from 8:30 am to 5:00 pm.
Request from attorneys and insurance companies have to be mailed or delivered in person; accompanied with the signed patient release form.
