Home Infusion Services
With the help of our expert clinical team, patients can receive intravenous (IV) medications at home.
Oncology Care
When a patient is referred, his or her physician works with our cancer care team to develop a home treatment plan. Our nurses are certified in administering the most complex intravenous therapies, including in-home chemotherapy, antibiotic therapy, total parenteral nutrition and pain management medications.
HIV+ and AIDS Care
Our pediatric and adult patients have access to the latest AIDS therapies. Home care team members, including registered nurses, licensed clinical social workers and pharmacists, all have advanced training in AIDS care and work closely with the patient's primary care team to coordinate treatment.
Cardiac Care
A cardiac clinical specialist provides one-on-one teaching to those with congestive heart failure or chronic obstructive pulmonary disease, as well as those who have had heart attacks or surgery or who require intravenous medications, such as heparin therapy. The goal is to avoid hospitalization by helping cardiac patients and their caregivers manage medications, diet and other aspects of treatment.
Ostomy, Wound Care and Continence Management
Patients and families can call our specialty team for training and assistance in tasks such as changing ostomy bags, dressing wounds and performing self-catheterization. Registered nurses with advanced certification have knowledge of the latest treatments and products and are available to consult with patients and their physicians.
Diabetes Management
Registered nurses trained in diabetes education monitor patients' health and help them and their families manage Type I, Type II and gestational diabetes.
Rehabilitation Services
Licensed therapists, all with graduate degrees, and therapy assistants provide physical, occupational and speech-language therapy to children and adults. Team members work with our medical equipment and pharmacy department, as well as our other clinical experts, to help patients become as independent as possible.
