After a hospital stay, transitioning to care at home can be challenging without careful preparation. Johns Hopkins Care at Home has onsite home care coordinators at The Johns Hopkins Hospital and Children’s Center, Johns Hopkins Bayview Medical Center, Johns Hopkins Howard County Medical Center and Anne Arundel Medical Center to provide discharge planning and home care coordination services before you leave the hospital. Our team handles the details for your home care needs, so you can focus on recovery.  

We also have liaisons onsite at Greater Baltimore Medical Center, Suburban Hospital and Sibley Memorial Hospital for adults; and at Children’s National Hospital, Kennedy Krieger Institute and Mt. Washington Pediatric Hospital for children. Our liaisons will coordinate home care for you, if you choose to receive care from Johns Hopkins Care at Home.
