Johns Hopkins Care at Home – Potomac Personal Care., a private duty agency, is a sister company to Johns Hopkins Care at Home – Potomac Home Health, and a service of Sibley Memorial and Suburban Hospitals. It is a non-profit company providing private pay services.

Using a team approach, we make every attempt to recognize and meet the needs of homebound patients.
Johns Hopkins Care at Home – Potomac Home Health
Our program stresses personalized care, tailored to the needs of each patient. We work closely with the physician, the patient and family members to plan, review progress and make adjustments to the plan of care. This approach results in a better treatment plan and greater understanding and support for patients and family members.

A registered nurse performs a comprehensive nursing assessment, develops a plan of care and coordinates care with the physician and other team members. The nurse performs nursing procedures and assists patients and caregivers to become independent in care by instructing them in the disease process, medications and symptom management.

Our team of professionals provides comprehensive infusion services including hydration, antibiotics, chemotherapy, total parenteral nutrition, pain management, blood products, catheter care and enteral nutrition. Our infusion nurses promote an environment conducive to the successful completion of therapy and the safe delivery of care.

A registered clinical dietitian assesses the patient's nutritional needs based on medical diagnosis and current habits. The dietitian then teaches the patient or caregiver to achieve the defined nutritional goals.

Licensed clinical social workers (LCSWs and MSWs) participate in the development and implementation of the plan of care by assessing social factors that impede recovery and coordinate use of community resources and special support groups. Social workers also counsel patient or family members if there is a need related to loss of health and function and/or a need for psychiatric services.

A licensed physical therapist assesses the patient's ability to function in the home by evaluating strength, range of motion, endurance, coordination, balance, pain, posture and mobility. The therapist develops and implements a rehabilitation program that will help the patient maximize total function.

A licensed occupational therapist assesses the patient's ability to perform activities of daily living and fine motor functions, the need for adaptive equipment in the home and the patient's cognitive and perceptual function. The therapist develops a program geared to maximizing safety and independence.

A licensed speech-language pathologist assesses and treats speech, language and swallowing disorders. Specific treatment programs are developed for aphasia, voice disorders, apraxia, dyspraxia, dysphagia, speech and language in order to maximize the patient's communication and/or swallowing skills.

Home care aides provide or assist patients with such personal care as bathing, dressing, oral hygiene, meal preparation, feeding, walking and exercise. These services are designed to help patients regain maximum independence in providing self care.

Johns Hopkins Care at Home – Potomac Personal Care provides your loved ones with personal attention and care at home or in the hospital. Our well-trained and experienced personnel are bonded and insured and are supervised by a licensed nurse.

Johns Hopkins Care at Home – Potomac Personal Care Services
Johns Hopkins Care at Home – Potomac Personal Care services are offered 24 hours a day/7 days a week, and include:
- Licensed nurses
- Home care aides
- Sitters/companions
- Mother’s helpers
- Live-ins
- Social worker available
Duties include:
- Personal care
- Newborn and child care
- Overnight care
- Meal preparation and assistance with feeding
- Reminder to take medications
- Assistance with bed mobility, transfers and ambulation
- Assistance with bowel and bladder elimination
- Accompaniment to doctor appointments
- Light housekeeping
- Shopping
- Communication of changes in client condition to the office
- Case management assessments
- Skilled nursing care and medication administration
The home care aide works in accordance with a written assignment prepared by a licensed nurse. The nurse makes an initial on-site visit once home care is started.
Payment Plans
Home care aide and sitter services are generally not covered by Medicare or other insurance companies. You may be asked to make an initial deposit and will be billed biweekly. You will receive an itemized bill for insurance or tax purposes. Please call for rate information.
Home health care services are covered by Medicare, Medicaid, Blue Cross, CHAMPUS and most commercial insurance policies. We also honor private-pay. To be eligible, patients must:
- Be confined to the home
- Require skilled nursing or therapeutic services on an intermittent basis
- Have a home health plan established by a physician

Your tax-deductible contributions to Johns Hopkins Care at Home – Potomac Personal Care and Johns Hopkins Care at Home – Potomac Home Health are used to provide services to individuals who are unable to afford home health care.
