Improving Geriatric Health Outcomes in Maryland

The Johns Hopkins Geriatric Workforce Enhancement Program (JHGWEP) is an educational initiative of the Division of Geriatric Medicine and Gerontology of the Department of Medicine of the Johns Hopkins University School of Medicine.

The purpose of the JHGWEP is to develop an integrated geriatric primary care health care workforce that maximizes patient and family engagement and results in improved health outcomes in Maryland. We propose to accomplish this by integrating geriatric principles into the workflow of selected community-based primary care practices, and by engaging older adults in their own health care by integrating and expanding the services of community-based health advocacy and educational organizations.
