Applicants are asked to submit the following items to the GIM Fellowship Coordinator:

• A completed application
• GIM fellowship Track Selection and Area of Interest form
• A curriculum vitae
• Copies of college, medical school and any advanced degree (e.g., master's degree) program transcripts (Please note that copies of these are sufficient; original transcripts are not required.)
• A one-page personal statement of research interests
• Four letters of recommendation (These should be sent by the authors directly to the GIM Fellowship Coordinator.)
