Prepare for an Appointment

Patients who have been contacted for an appointment within our center will receive a new patient package in the mail. We ask that you complete the new patient questionnaire in that packet prior to coming to your scheduled visit and bring it with you to your appointment.

If this is your first visit, please bring all:

• Pertinent physician records
• Lab reports
• X-ray films
• CT or MRI films (if available)

In addition, if you have been referred by a physician, please bring the complete name, address, and phone number of your referring physician.

We ask that you arrive 15 minutes prior to your scheduled appointment time to complete any necessary registration information.

If you must cancel this appointment, please call us at 410-955-5810 at least 48 hours in advance. We will happy to schedule your appointment at another convenient time.
