Prepare for an Appointment

Once you receive an appointment date, please plan to arrive in the clinic at least 30 minutes in advance to check in.

If a patient questionnaire was sent to you, please bring the completed form with you.

Your physician will likely want to see any imaging studies (CT scans or MRI scans) of your brain or spine so please arrange to have the films or images sent on discs (not just the paper reports) or you may bring them along with you to your appointment.

The results of any tests that may not have been sent to us should also be brought along.
