Advantage MD D-SNP Provider Training
The Centers for Medicare and Medicaid Services (CMS) requires all providers who interact with Johns Hopkins Advantage MD D-SNP (HMO) members to complete initial training on the annual Model of Care (MOC) and then annually thereafter and attest to its completion.
The training will describe Special Needs Plans (SNPs), define the Model of Care, as well as explain provider responsibilities and member benefits.
All D-SNP providers are required to complete this training initially when they join the Advantage MD D-SNP network AND annually, especially on the care teams of D-SNP members.
Step 1: Complete the mandatory training
Register for and attend one of the training sessions below OR review the Advantage MD Provider Education Presentation – D-SNP (HMO).

After registering for the session, you will receive a confirmation email containing information about joining the meeting and the ability to add the invitation to your calendar.
Step 2: Submit the attestation form
The course you have just completed fulfills the CMS requirement. You are asked to attest that you understand the concepts and responsibilities of the Johns Hopkins Advantage MD D-SNP (HMO) plan MOC.
I hereby attest that I have completed the required MOC training course and that I understand the MOC key concepts below:
- I understand the population of members that are defined as Special Needs and enrolled in the Johns Hopkins Advantage MD D-SNP (HMO) plan.
- I understand Care Coordination, its principles and my responsibilities as a provider.
- I understand that quality and performance measurement is a key piece of the program and I understand my responsibilities as a provider to participate in the Johns Hopkins Advantage MD D-SNP (HMO) program(s).
- I understand my role and responsibilities as a participating network provider.
- I understand all identified updates and changes made to the annual MOC.
