After completing required forms, you will mail us copies of pertinent diagnostic imaging tests. A fee will be charged to a credit card prior to the images being reviewed. A formal letter containing the Johns Hopkins physician’s assessment and recommendation(s) will be mailed or faxed to your referring physician.
