Requesting an Appointment

Patients Self-Referring

Self-referring patients are asked to call the Johns Hopkins Hospital Sleep Disorders Center at 443-287-3313 for the Outpatient Clinic location, and 410-550-0571 for the Johns Hopkins Bayview Medical Center location.

If you have not previously been a patient of Johns Hopkins you will need to be registered before making the appointment, please have your insurance cards ready for this. Patients will then be asked a set of questions to aid the scheduler in matching the appropriate doctor, with a specific specialty, which is most likely to help with your sleep problems.

New patients should bring in a completed Sleep Questionnaire and Sleep Log to their clinic visit.

Second Opinions

Any patients looking to schedule an appointment for a second opinion should state so during your call as any previous sleep tests, clinic notes, or any other sleep documentation will be requested to be sent prior to scheduling your appointment so our physician may review before deciding which specialist is right for you to be scheduled with.

Doctor Referring a Patient

Johns Hopkins' physicians should create an order in Epic and specify which sleep center location they wish to send the patient in order for it to be processed in the correct work queue.

We ask all outside referring providers to complete the following Request Form for patients you are looking to schedule a clinic visit. If you are requesting a sleep study be done prior to an initial consultation, the request form must be completed along with a copy of your most recent clinic note, including a history and physical with documentation on the necessity for a sleep study, and any necessary insurance referrals. These should be faxed to the scheduling staff at 410-550-2550 where it must be reviewed and approved by our physician.
