Johns Hopkins Treatment Resistant Esketamine Antidepressant Targeted (TREAT) Depression Clinic

The Johns Hopkins TREAT Depression Clinic offers screening services to evaluate if esketamine treatment would be appropriate for persons diagnosed with treatment resistant depression and esketamine treatment services for those evaluated to benefit from the treatment.

Screening Visit Process

If you have been referred by your provider to explore esketamine treatment, please complete the following information to schedule a screening visit and send either by mail or via e-mail using the contact information below.

  1. The following completed new patient forms and questionnaires:
  2. A referral from your psychiatrist. The referral needs to state that your psychiatrist is referring you for this adjunctive treatment and will continue to serve as your primary psychiatrist. It should include a brief psychiatric history, medication trials and results, and that you meet the diagnosis of treatment resistant unipolar depression. We currently are not treating patients with Bipolar Depression (any history of manic episodes), active substance use (there is a minimum if 30 days of no substance use before we can consider treatment), or MDD with psychosis.
  3. A list of your past psychiatric medications (you can get this from your pharmacy).
  4. A copy of your insurance card.

Once the patient information package is complete and received, we will set up a screening visit for you to meet with one of our clinic psychiatrists, which may be either a virtual or in-person appointment. During the screening visit, the psychiatrist will review your psychiatric history, assess your need for treatment, and if appropriate, prescribe and seek approval from your insurance company for esketamine treatment to be done in the clinic.
