On January 1, 2019, JMAP entered into an agreement with the Centers for Medicare and Medicaid Services (CMS) to participate as a Care Transformation Organization (CTO) in the Maryland Primary Care Program (MDPCP), an initiative designed to provide patient-centered care to Medicare fee for service (FFS) patients who live in Maryland.
The CTO is a new type of entity that hires and manages an interdisciplinary care management team to furnish an array of care coordination services. The CTO will utilize JMAP’s existing infrastructure to offer customizable care delivery, information technology, and quality improvement solutions to partner practices to realize value in care transformation.
