Telegenetics
ARVC is often caused by a genetic mutation that you may not know you have, so early genetic testing can have lifesaving benefits. Using telegenetics, we connect you with Johns Hopkins ARVC specialists where you are. This means you can stay home and speak to a genetics counselor from Johns Hopkins. We also provide follow-up recommendations to your local doctor or hospital.
After review of your medical records, family history and any other pertinent information, our genetics counselor will determine the most appropriate genetics test and discuss the risks, benefits and limitations of the specific testing recommended.
If you want to set up an appointment with our team, please print and complete the following forms:
- Consultation Request Form
- Patient Intake Form
- Patient Disclaimer
- Medical Records Release
- Payment Authorization
- Genetic Counseling Consent Form
Submit completed forms by email or by mail/courier to:
