In-Home Primary Care
Johns Hopkins Home-Based Medicine is an in-home primary care program for adults age 65 or older who are unable to leave their homes due to medical conditions or physical limitations.
Participants benefit from our team of experienced geriatricians, nurse practitioners, registered nurses, social workers and community health workers. Our highly skilled team works with patients to develop care plans that meet their individual needs.
Services We Provide
We visit patients in their homes and call regularly to check in and assist with the following:
- Primary care designed for older adults
- Medication review
- Behavioral health assessment and therapy
- Managing chronic health conditions
- In-home vaccinations and blood work
- Coordinating community resources for patients and caregivers
We can also provide medical support by phone – 24 hours a day/seven days a week – and urgent visits as needed.
To participate in the program, patients must live in certain areas of Baltimore City, Baltimore County or Howard County (see zip codes served below), they must be age 65 or older, and must have a chronic disease or advanced illness that prevents them from leaving home or driving.
Participants must be able to use the telephone, or have a caregiver to assist with scheduling appointments. They must also have either Medicare or the Johns Hopkins Medicare Advantage plan and must use a Johns Hopkins Home-Based Medicine physician as their primary care doctor.
