Hopkins Providers

***JHG will pick up samples from CMSC 1004 on Tuesdays and Thursdays by 10am. A JHG sample pickup box will be brought to the GC suite on Monday 11/25.

Clinical Exome Sequencing – INPATIENT

  1. Complete the Inpatient Exome requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
  2. Place an Epic order (JHG Clinical Exome Sequencing).
    1. If blood is selected as the sample type, this order will prompt a proband blood draw in Epic.
    2. If you need saliva kits, please let Brittany and/or me know.
    3. If you have proband or parental saliva samples to be picked up, please place the samples in the “Johns Hopkins Genomics – Sample Pickup” box that will be placed in the GC suite. Samples from this box will be routinely picked up by a member of the JHG staff throughout the week.
  3. If this is a RUSH case (i.e. you would like verbal results in 21 days) provide a brief summary of the patient’s clinical presentation and reasoning for rushing the exome to Brittany and me. We will review this with our technical leadership to ensure that we are in a place amendable to accepting a RUSH case.

Clinical Exome Sequencing – OUTPATIENT

  1. Initiate the pre-authorization process by contacting the JHG billing coordinator, Sabrina Ingram singram1@jhmi.edu.
    1. Please provide the test you would like to order, the ordering provider, the patient’s name, MRN, DOB, and insurance provider in this email to ensure a quick initiation of the pre-auth process.
  2. Complete the requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
  3. Place an Epic order (JHG Clinical Exome Sequencing).

Zoom Tests

  1. Initiate the pre-authorization process by contacting the JHG billing coordinator, Sabrina Ingram singram1@jhmi.edu.
    1. Please provide the test you would like to order, the ordering provider, the patient’s name, MRN, DOB, and insurance provider in this email to ensure a quick initiation of the pre-auth process.
    2. If the patient is inpatient when this test is ordered, pre-authorization is not required to initiate testing.
  2. Complete the requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
    1. Indicate which subcategory(s) you would like to order for this patient.
  3. Place an Epic order (JHG Zoom).
    1. In the comments section, indicate which Zoom you are ordering.
    2. If blood is selected as the sample type, this order will prompt a blood draw in Epic which can completed at Express testing.
    3. Note: we do allow saliva as a sample type for this test (reach out with questions)
    4. If you have proband or parental saliva samples to be picked up, please place the samples in the “Johns Hopkins Genomics – Sample Pickup” box that will be placed in the GC suite. Samples from this box will be routinely picked up by a member of the JHG staff throughout the week.

ZoomOut Testing Initiate the pre-authorization process by contacting the JHG billing coordinator, Sabrina Ingram singram1@jhmi.edu.

  1. Please provide the test you would like to order, the ordering provider, the patient’s name, MRN, DOB, and insurance provider in this email to ensure a quick initiation of the pre-auth process.
    1. If the patient is inpatient when this test is ordered, pre-authorization is not required to initiate testing.
  2. Complete the requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
  3. Place an Epic order (JHG Zoom Out).

Targeted Testing ($350/patient regardless of number of targets, all genes available)

  1. Complete the requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
  2. Place an Epic order (JHG Targeted Variant).
    1. If blood is selected as the sample type, this order will prompt a proband blood draw in Epic which can completed at Express testing.
    2. If you need saliva kits, please let Brittany and Jordan know.
    3. If you have proband or parental saliva samples to be picked up, please place the samples in the “Johns Hopkins Genomics – Sample Pickup” box that will be placed in the GC suite. Samples from this box will be routinely picked up by a member of the JHG staff throughout the week.
  3. VUS RESOLUTION TESTING: JHG will provide free testing for VUSs (identified by a JHG test) in potentially informative family members identified. Please contact jbrow290@jhmi.edu or bbrasse1@jhmi.edu to coordinate this prior to sending a sample.

Prenatal Targeted Testing ($1350/order regardless of number of targets, all genes available, includes MCC)

  1. Complete the requisition form and scan into Epic under Media or email the completed requisition form to jbrow290@jhmi.edu or bbrasse1@jhmi.edu.
  2. Place an Epic order (JHG Prenatal Targeted Variant)
    1. Please note the prenatal sample types accepted on the attached cheat sheet.
    2. If blood is selected as the sample type, this order will prompt a maternal blood draw in Epic which can completed at Express testing.
    3. If you need saliva kits, please let Brittany and Jordan me know.
    4. If you have parental saliva samples to be picked up, please place the samples in the “Johns Hopkins Genomics – Sample Pickup” box that will be placed in the GC suite. Samples from this box will be routinely picked up by a member of the JHG staff throughout the week.
