Johns Hopkins remains committed to leveraging the important work and learnings of J-CHiP across the delivery system through future programs to improve care coordination.
For many people, the road to better health is not as simple as one visit to the doctor. The Johns Hopkins Community Health Partnership (J-CHiP) is committed to helping patients in East Baltimore to get and stay healthy through comprehensive care that is easy to access.
Whether it’s a visit from a home care nurse or community health worker, help with getting medications and keeping them straight, counseling or a neighborhood fitness program, the program is designed to provide it through a team of health care workers who coordinate care as needed. The program not only enables residents to work toward good health, it can also help prevent unnecessary hospital stays and keep costs down.
